Register for CVC in San Diego November 6-9th, 2009 Click here to download the registration form. Early registration may also secure your seat in popular Wet Labs, tours, and other programs. Registrations postmarked after September 30, 2009, will be processed at the higher, on-site rates and may not be available for immediate pickup on site. Register online or fax or mail the registration form. All essential information must be provided in order for the registration form to be processed. The registration form is for one attendee and guests. Use a separate registration form for each attendee, making copies as needed. Everyone attending CVC in San Diego and requesting hotel accommodations must be registered. Write your name as you wish it to appear on your badge and in the registration roster. Guests only have access to the Exhibit Hall. Hospital team members are not considered guests. All veterinarians, veterinary technicians, practice managers, and team members must register and pay registration fees. Exhibit Hall-only badges for team members may be purchased ($30 each on-site). Registrations made using a purchase order will be accepted in lieu of payment to hold your place. All fees must be paid by September 30, 2009, to receive the preregistration rates, and all payments must be received before the start of the conference. Wet Labs and Workshops require an additional (nontransferable) registration fee. CVC reserves the right to cancel any lab that does not meet minimum registrations by October 7, 2009. If the lab is cancelled by CVC, registrants will be notified and all paid fees credited or refunded. All cancellations of a lab, workshop, or registration by the registrant must be submitted in writing for a credit or refund minus a $50 processing fee. No requests for refunds or changes will be accepted after October 7, 2009. Licensed child-care services will be offered Friday through Monday in the convention center. Sign up for child care on the registration form and indicate the name and age of each child. You will receive a packet of information and forms for each registered child. Child care is limited and confirmed on a first-come, first-served basis. Cancellations must be made in writing and received no later than October 7, 2009.
